So we are thinking about doing a "questionnaire" instead of a guestbook, that way everyone can do it at their own table and write out thoughtful answers (hopefully)

we need some questions to ask...

either about the guest themselves
advice

joanna goddard had a series on her blog :
what's your secret to a happy marriage

so that will be one question, but can you think of anymore?

We did that too. Here (http://forums.onewed.com/showthread.php?t=24208&highlight=guestbook) is the one I made. It was really fun reading what people wrote. Not everyone filled them out though since some people came to the wedding but not the reception and a few people were just too stuck up to fill them out. But we had a guestbook frame also so between the pages & the frame, I think we got everyone!

The one thing that really bummed me out is that no one handed them out to the head table so our closest friends didn't fill one out!

Ours asked:

Guest to sign their name
How they know bride & groom
Where they traveled from
Any advice for the newlyweds
Favorite memory of bride and/or groom
Special wishes for us
Just for fun - draw us a picture

The picture was my favorite part. Some friends of DH drew a few naughty pictures on some by the end of the night but we just threw them out. Overall it was a good idea & I was glad we did it.

We haven't done anything with the pages yet...they are buried in a box from when we moved. I really need to put them in a scrap book or something!

I know its not DIY, but we used these. Everyone loved them and we only used a page per family so there was not many to buy.

How about:

"Always Remember/Never Forget"
"How To Get Along With The In-Laws"
"Ways To Make Up After A Fight"

I love this idea! The examples shown appear to be standard 8 1/2 X 11 and could be punched with three holes and filed into a pretty binder or photo album. Great alternative to a guest book where people may be just signing in because they're in a hurry to get to their table.

We did that as a guestbook alternative too. Here is how my pages came out -

We had a lot of fun answers, and some really cute drawings too. A lot of people drew pictures of our decorations and centerpieces, which is a fun way to remember them. :)
